Smoking’s effects on lung and heart health are well documented territory, but its effect on sperm gets less attention despite a fairly consistent research base.

The Concentration Gap

A May 2024 study published in Frontiers in Endocrinology compared semen samples from heavy smokers and nonsmokers among men being evaluated for infertility, and found heavy smokers had a median sperm concentration of about 59 million per milliliter, compared with about 69 million per milliliter in nonsmokers, a difference of roughly 14%.1 It’s one data point among a broader research base that has consistently linked smoking to lower sperm concentration across multiple studies and decades.

Why the Mechanism Matters

Cigarette smoke introduces a mix of chemicals linked to oxidative stress, one of the more consistently cited mechanisms behind smoking’s effects on sperm - oxidative stress can damage sperm cells during production and reduce motility and concentration in the process. It’s a similar biological pathway to the one researchers point to for several other factors covered in this guide.

A Modifiable Factor

Unlike genetics or age, smoking is one of the more directly modifiable lifestyle factors in this research base, and its effects on general health extend well beyond reproductive health specifically.
